personnel shall confirm that the following is fulfilled:

• I confirm that during the past 10 days I have NOT visited or travelled through any country outside of China.

• I confirm that I have not been in contact with people that is confirmed affected by the Corona virus within the last 10 days.

• I confirm that I have not experienced ANY symptoms related to the Corona virus; cough, fever or wheezing / shortness of breath

within the last 24 hours.
